Massachusetts Society for the Prevention of Cruelty to Children (MSPCC) is seeking a full-time clinical supervisor providing skill-based, clinical and administrative supervision of In-Home Therapy, Therapeutic Mentoring, Therapeutic Training and Support, and Outpatient Clinicians, as part of the Children's Behavioral Health Initiative (CBHI). CHD is seeking a full-time, 40-hour per week, RN to work within the CBFS system that coordinates functions and activities that provide for, maintain, and promote the health of clients in need of Department of Mental Health funded services. Baystate Pediatrics - A Baystate Children's Hospital Practice is part of Baystate Health's board certified team of pediatricians, many of whom are nationally recognized for their expertise in patient care, clinical research and medical education.  We provide primary care and hospital coverage to patients throughout western New England.

 

The Community Liaison Outreach Specialist coordinates initiatives designed to promote the visibility of the health center and its services to the community. Administers organizational programs aimed at addressing the needs of the surrounding area. Develops relationship with community leaders and serves as the health center's liaison with various constituents.

Additionally the Community Liaison Outreach Specialist will serve as a liaison to the community to assist and coordinate patient access to available health center services, follow-up and locate patients lost to care, as directed by supervisor, attend and/or serve on various committees/boards of constituent organizations to promote visibility of health center and its services as well as plan & implement community outreach activities in consultation with Practice Manager.

 

Qualifications

 

  • 2 - 4 years experience in health outreach and community services within low-income communities required. Bachelor' s Degree preferred
  • Computer literate in MS Word, PowerPoint and Outlook.
  • Must possess excellent presentation skills and interpersonal skills.

Bilingual Outreach Specialist/Health Educator- Prenatal & Newborn Medicine - FT


Baystate High Street Health Center Pediatrics provides comprehensive pediatric care in an urban community health center.  On a journey to becoming a patient centered medical home, the mission of BHSHC is to improve the lives of our patients and our community every day by providing health care in an environment that values quality, teamwork, innovation, cultural diversity, educational excellence, and mutual respect.

 

Through a presence on the Labor and Delivery units and in the Continuing Care Nursery (CCN), High Street Health Center Pediatrics Prenatal & Newborn Outreach Specialist (PNOS) will assist families with linking their newborns to the HSHC Patient Centered Medical Home.  


The PNOS will participate in rounding in the nursery and CCN with the pediatric team under the supervision of the Newborn Nursery Director.   The PNOS will provide educational materials and support for families who are planning to or are considering bringing their newborn to HSHCP. This educational support will include information about newborn care, breast feeding, and services available at and through HSHCP, and will include education about the Medical Home concept and the importance of primary care for the health and well-being of their newborn.  The PNOS will be involved in pre-natal and newborn educational sessions in the BMC nursery, at HSHCP, and in the community. When appropriate, the PNOS will work with BMC lactation consultants, the HSHCP Community Liaison, and others to coordinate these efforts. The PNOS will also track enrollment of newborns, patient satisfaction and other data improving on baseline data. 

 

Qualifications:

  • High school education or equivalent. 
  • 2-4 years of experience working in community health setting in a similar role such as health educator, outreach worker or medical assistant.  Experience working in  maternal/newborn medicine preferred 
  • Computer literate in MS Word, Excel and Outlook.
  • Excellent interpersonal and customer service skills.  Must have ability to effectively communicate orally & in writing with diverse community and health center staff.
